The form can also be used for pre-evaluation at the Department of Pharmaceutical Sciences
Use this form for:
- Request to transfer from other universities
- Transfer from one degree programme to another
- Recognition of certificates achieved abroad
- Return to study
- Recognition of certification, etc.
Application to modify Study Plan
The following form to apply for a modification to the Study Plan:
- Completed within the deadlines stated in the section Study Plans: general information
- Be signed by your teacher-tutor,
- Be sent to the secretariat at: allegati.disum@uniupo.it.
